Rejection Criteria
This sample will be rejected if not labeled properly:
- Must be labeled with patient LAST name, FIRST name, and MRN.
- All blood bank samples must have HAND WRITTEN collection information on the label to be accepted (date, time, and initials).
- All information must be legible.

Additional Information
- Antibody screen sample, including but not limited to, Type and Screen, will be used.
- Additional sample will be requested if quantity not sufficient.
Clinical Indications
When antibodies are detected in a blood bank screening test (such as Type and Screen), additional testing shall be performed to identify antibodies of clinical significance. In a patient with a history of antibodies, additional antibody identification testing is performed to detect and identify newly formed clinically significant antibodies. All clinically significant antibodies detected must be identified prior to transfusion and when indicated, antigen negative and crossmatched red cell units are provided.
