Test Code LABAPCONSULT Pathology Consult - HP

Important Note

This order is strictly for UCM providers requesting in-house pathology review of outside hospital pathology materials (referral slides/blocks) for patients that are transferring their care to UCM.

This order is not to be used for specimens collected in UCM operating rooms, procedural or clinic areas.

 

 

Specimen and Container/Tube

All slides should be packaged in protective hard plastic slide mailers along with the relevant pathology reports and clinical paperwork. 

Specimen Handling and Transport Instructions

UCMC Onsite Instructions if patient brings materials to their appointment: 

  • Enter Pathology Consult - HP order as a Normal order
  • Fill out the fields
  • Sign the order
  • Deliver the materials to Pathology Client Services, room J-601 in SBRI

UCMC Onsite Instructions if materials are requested outside of the patient's appointment: 

  • Navigate to the Orders Only activity in EPIC.
  • Create a NEW encounter (encounter must have a billable HAR associated with it).
  • In the Orders activity, place an order for Pathology Consult - HP as a Future order.
  • Fill out the fields in the order and accept.
  • Sign the order.
  • Associate the order with an appropriate diagnosis and accept.
  • Sign the encounter.
  • Request that materials be sent via FedEx to:

UCMedLab Outreach Instructions: Pathologists from outside institutions requesting a personal consult

  • Contact UCMedLabs or Pathology Client Services at 773-702-8287 in order to set-up a Client Account.
  • Once an account is made a requisition form unique to your institution, including your client code, will be sent to you with instructions on how to package and send materials to UCM.
  • A cover letter and all relevant reports and materials shoul be sent via FedEx to:

Turnaround Time

Turnaround times are relative to the time the speimen is received in the department. It also varies depending on complexity of case, need to request additional materials from the outside hospital, and performing additional staining. 

Department hours: Monday - Friday; 8am - 5:30pm

Varies: 88321, 88323, 88325 and codes for staining, if performed.
