Specimen Handling and Transport Instructions

UCMC Onsite Instructions: 

Collect specimen per standard collection procedure and send to laboratory immediately. 

 

UCMC Offsite and UC MedLab Outreach Instructions: 

  • Samples should be centrifuged within 2 hours of collection.
  • Centrifuge specimens at >2500 x g for 10 minutes at room temperature.
  • Aliquot sample into plastic transport tube containing a minimum of 0.2 mL of specimen.
  • Samples must be sent room temperature at 15-25oC.

Rejection Criteria

  • Common specimen rejected situations include: incorrect specimen type, insufficient volume, missing or incomplete specimen identifiers, incorrect specimen transport, or specimens outside stability limits. 
  • Specimens that are clotted or grossly hemolyzed.
  • Specimens that were transported unspun/whole blood and draw time is greater than 6 hours.

Glomerular Filtration Rate (eGFR)

Clinical Indications

  • Serum creatinine is the most commonly used marker for estimation of GFR. However, it has become evident that the creatinine concentration is far from ideal because it is significantly changed by other factors such as muscle mass, diet, gender, age and tubular secretion.
  • Cystatin C is a low-molecular weight protease inhibitor that is constitutively expressed by all nucleated cells and freely filtered by the kidneys. Serum/plasma concentrations of Cystatin C is inversely correlated with GFR.
  • Cystatin C in plasma and serum has been proposed as a more sensitive marker for GFR in children and adults, and several studies, as well as one meta analysis, have suggested that cystatin C is superior to serum creatinine for estimation of GFR.

  • Results should always be assessed in conjunction with patient's medical history, clinical examination and other findings.
  • Cystatin C-based estimated GFR is calculated using the Schwartz Cystatin C equation for patients under 18 years old and the CKD-EPI Cystatin C Equation (2012) for patients 18 years of age and older.
